When a customer requests any work relating to forms i.e. form creation, form modification, it is necessary for them to fill in a form template document.

The aim of this document is to allow the customer to accurately convey requirements for form creation to us.

It also provides us with a document to refer back to should any confusion arise.

When working on form creation / form modification it is important to follow the form checklist.

This step is in place to ensure the users are accurately tested and verified before informing the customer that their changes have been made.
