===== FA Code =====
  * FA codes are used to identify equipment for traps forwarded to AT&T.
  * The FA code refers to the carrier_hub_id in the network_element table.
  * FA codes are set at the Controller level

Traps are currently forwarded to AT&T if:
  - Severity is Critical or Major
  - And Element has FA code (carrier_hub_id) assigned to them or their parents
  - And assigned Carrier is AT&T or OPN

  * In the trap forwarding query FA codes are referenced as fa_location_code
  * In the trap forwarding conversion FA codes are referenced as equipmentIdentifier
